Although inhaled nitric oxide therapy is the treatment of choice for uncomplicated persistent pulmonary hypertension of the newborn PPHN ; , metabolic alkalinization remains an important initial resuscitative strategy during shock in neonates. PPHN in the setting of SS can be reversed when acidosis is corrected. For centers with access to inhaled nitric oxide, this is the only selective pulmonary vasodilator reported to be effective in the reversal of PPHN. It is urgent today to shed light on what really happened since the putsch of 11 January 1992, for civil peace and stability will only be possible once the victims' suffering is recognised, those responsible for the crimes named and a process of reconciliation based on truth and justice initiated. Drug-induced hepatotoxicity requires the involvement of Kupffer cells, which are responsible for mediating the inflammatory processes through the release of several pro-inflammatory cytokines such as IL-1 , TGF- , TNF and IL-6. Indeed, inactivation of Kupffer cells by in vivo treatment of rats with gadolinium chloride prevents the toxicity exerted by e.g. paracetamol Michael et al. 1999 ; and carbon tetrachloride Muriel and Escobar 2003 ; . SHOULD BE EVALUATED FOR AN ENLARGED SPLEEN OR SPLENIC RUPTURE. Acute Respiratory Distress Syndrome ARDS ; Acute respiratory distress syndrome ARDS ; has been reported in patients receiving Neulasta, and is postulated to be secondary to an influx of neutrophils to sites of inflammation in the lungs. Patients receiving Neulasta who develop fever, lung infiltrates, or respiratory distress should be evaluated for the possibility of ARDS. In the event that ARDS occurs, Neulasta should be discontinued and or withheld until resolution of ARDS and patients should receive appropriate medical management for this condition. Allergic Reactions Allergic reactions to Neulasta, including anaphylaxis, skin rash, urticaria, and erythema flushing have been reported in postmarketing experience. The majority of reported events occurred upon initial exposure. In some cases, symptoms recurred with rechallenge, suggesting a causal relationship. In rare cases, allergic reactions including anaphylaxis, recurred within days after initial anti-allergic treatment was discontinued. If a serious allergic reaction occurs, appropriate therapy should be administered, with close patient follow-up over several days. Neulasta should be permanently discontinued in patients with serious allergic reactions. Sickle Cell Disorders Severe sickle cell crises have been associated with the use of Neulasta in patients with sickle cell disorders. Severe sickle cell crises, in some cases resulting in death, have also been associated with Filgrastim, the parent compound of pegfilgrastim. Only physicians qualified by specialized training or experience in the treatment of patients with sickle cell disorders should prescribe Neulasta for such patients, and only after careful consideration of the potential risks and benefits. The receptor subtypes implicated in the nonneural 5-HT-mediated colonic epithelial response remain controversial. In 1993, Siriwardena and Kellum 46 ; confirmed their previous results 44 ; concerning involvement of 5-HT2 receptors in the nonneural pathway of 5-HT-induced electrolyte transport in the rat distal colon. Other studies have shown a role for 5-HT4 receptors 1, 5, 6 ; in 5-HT-stimulated Isc responses together with an insensitivity to 5-HT1, 5-HT2, and 5-HT3 receptor antagonists 6 ; . Our study supports evidence in favor of the involvement of 5-HT4 receptors, as proposed by Bunce et al. 6 ; and as shown by Budhoo et al. 5 ; and more recently by Albuquerque 1 ; . Indeed, colonic epithelial responses to 5-HT were totally abolished by the 5-HT4 receptor antagonist SDZ-205, 557 and were insensitive to TTX, thus confirming a 5-HT4mediated response of nonneural origin. Zimmerman and Binder 49 ; have also shown responses to exogenous 5-HT to be insensitive to TTX in rat distal colon. Similar data have also been reported for intact mouse colon ENS intact ; , in which TTX had no effect on 5-HT-stimulated Isc responses 23 ; . One explanation for such data may be that 5-HT receptors are on TTXinsensitive nerves. In the light of such findings, further experiments using other inhibitors of neurally mediated responses such as hexamethonium, capsaicin, or lidocaine in both stripped and intact tissues are necessary. In irradiated tissues, the present data suggest that responses to exogenous 5-HT still occur via the same pathway, involving 5-HT4 receptors. However, implication of the same receptor subtype does not rule out the fact that the characteristics of these receptors may be modified following radiation exposure. The decreased 5-HT EC50 values, compared with those obtained in sham-treated animals, observed in the present study could be due to changes in mucosal-submucosal tissue.
